Important Information for the Upcoming Graduating Class

Once you have submitted your Application to Graduate please make sure that you are aware of how your name will appear on your diploma.

If your name is spelled incorrectly or you wish to add an accent mark or additional name please fill out and return to the Records Department the Current Student Diploma Name Change form which can be found on the web at www.uic.edu/depts/oar/forms/csdnchange.pdf .

All name change information must be submitted to the Records Department of OAR no later than the last day of term for all colleges.
